Job Summary:
The TreatmentNavigator supports in the recovery and capacity building of clients tomaximize their social and economic potential and support their goals for successwithin a stay in addictions treatment.
This role works as part of a multi-disciplinary team andfocuses their work with the Youth and Young Adult Population within ourtreatment programs.
Responsibilities:
Participatein team efforts to maintain and continuously improve the intake, admissions andorientation process. Maintainclient confidentiality and respond to client rights to health care information. Assessclients’ suitability for the program and makes referrals to other agencies whennecessary. Workwith the integrated addiction services team on clients’ case planning andconcerns. Ask for assistance when required, communicating with case managers,social workers, probation services, etc. as required. Maintainaccurate client records (e.g., assessment data, client care plan, goal setting,case notes and discharge summaries, behavior contracts, incident reports,medication logs, etc.). Actas a positive role model, coach, advisor, leader and rational authority forclients. Intakenew clients and collaborate with other staff members to identify acute andchronic mental health problems faced by clients. Develop,individually and in consultation with the primary treatment providers,treatment plans and strategies to best meet client needs. Assistclients with the completion of instrumental daily activities, includinghousehold upkeep and community participation. Observeclient behavior and maintain appropriate documentation. Report changes inphysical or mental health to the clinical team.Qualifications:
Grade12 plus graduation from a recognized Mental Health Care Worker Program or anequivalent combination of education, training, and experience. 1year of experience working with vulnerable populations. ValidClass 5 driver’s license with clean driver’s abstract. FirstAid Certificate with CPR: current. ValidFoodsafe certificate. Satisfactorycompletion of Employee Immunization Record form is required.Skills & Knowledge:
