**Position Description**

The Community Support Services Coordinator is a member of Kenora Chiefs Advisory’s Community Health and Social Services team. This position will provide liaison, support and case management for the First Nation Home Maintenance & Repair and supporting the Homemaker Programs, ensuring the needs of the elderly are being met.

Position Title: Community Support Services Coordinator

Department: Community Health and Social Services

Term: Full time

Location: 240 Veterans Drive, 3rd Floor, Kenora, Ontario

Reports to: Director of Services--Community Health and Social Services

**Summary of Duties**

Ø Work as part of the Health Team

Ø Provide direction and support to the Community Coordinators and/or Social Services Administrators in the First Nations

Ø Complete client assessments on home maintenance and repair and homemaker clients

Ø Maintain and update client files on a regular basis

Ø Advocate and navigate services for clients and health care providers in communities and First Nation members in Urban area

Ø Network with local specialized service providers

Ø Connect and work collaboratively with community health service providers to ensure efficient service and program delivery

Ø Maintain communication with community coordinators from participating First Nations ie: quarterly meetings, monthly visits or as needed

Ø Ensure accurate reporting of statistics and funding

Ø Complete the necessary research and proposals for expansion programs, training and services

Ø Obtain and complete quarterly reports and process monthly payments

Ø Other duties as required

**Qualifications**

Ø Completion of a post-secondary program that relates to this field or equivalent combination of training and experience

Ø Current knowledge and understanding of Federal and Provincial health programs

Ø General understanding of First Nation culture, values and beliefs

Ø Ability to work independently and in a team environment

Ø Excellent written, oral, interpersonal and presentation skills

Ø Excellent computer skills

Ø Fluent in Anishinabe language is a definite asset

Ø Experience working in a First Nation health organization

**Additional Requirements**:
Ø Valid driver’s license and vehicle.

Ø Must be willing to travel

Ø Vulnerable Sector Criminal Records Search.
