We are seeking a highly skilled and compassionate Nurse Practitioner to join our Palliative Care team at Alberta Health Services. As a Palliative Care Nurse Practitioner, you will play a vital role in providing comprehensive care to patients and their families, utilizing your expertise in clinical practice, leadership, and education.
Key Responsibilities- Provide advanced practice nursing care to patients with complex needs, including palliative care, in a variety of settings.
- Develop and implement individualized care plans, incorporating evidence-based practices and patient-centered care.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Conduct assessments, diagnose, and manage patients' health needs, including ordering and interpreting diagnostic tests, prescribing treatments, and performing procedures.
- Utilize advanced practice nursing competencies to optimize health systems, promote quality care, and inform healthcare policy.
- Participate in education and research activities to enhance palliative care knowledge and practice.
- Master's degree in Nursing or related field.
- Registration with the College of Registered Nurses of Alberta (CRNA) as a Nurse Practitioner.
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
- Minimum 5 years of clinical experience as a Nurse Practitioner, with at least 1 year of experience in palliative care.
- Experience in at least two (2) care sectors, including Home Care.
- Advanced clinical and assessment skills, with proficiency in MS Office Suite, TEAMs, ZOOM, and tele-health knowledge.
- Valid driver's license and access to a motor vehicle in good repair for work-related travel.
- CNA Certification in Palliative Care.
- McMasters Case Management Education.
- Meditech and Connect Care Trained.
- Previous experience in leadership roles, program planning, education of adults, interdisciplinary practice, and research activities.
- Competitive hourly rate: $62.74-$72.00.
- Full-time, permanent position.
