About the Role

We are seeking a highly skilled and compassionate Nurse Practitioner to join our Palliative Care team at Alberta Health Services. As a Palliative Care Nurse Practitioner, you will play a vital role in providing comprehensive care to patients and their families, utilizing your expertise in clinical practice, leadership, and education.

Key Responsibilities
  • Provide advanced practice nursing care to patients with complex needs, including palliative care, in a variety of settings.
  • Develop and implement individualized care plans, incorporating evidence-based practices and patient-centered care.
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
  • Conduct assessments, diagnose, and manage patients' health needs, including ordering and interpreting diagnostic tests, prescribing treatments, and performing procedures.
  • Utilize advanced practice nursing competencies to optimize health systems, promote quality care, and inform healthcare policy.
  • Participate in education and research activities to enhance palliative care knowledge and practice.
Requirements
  • Master's degree in Nursing or related field.
  • Registration with the College of Registered Nurses of Alberta (CRNA) as a Nurse Practitioner.
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
  • Minimum 5 years of clinical experience as a Nurse Practitioner, with at least 1 year of experience in palliative care.
  • Experience in at least two (2) care sectors, including Home Care.
  • Advanced clinical and assessment skills, with proficiency in MS Office Suite, TEAMs, ZOOM, and tele-health knowledge.
  • Valid driver's license and access to a motor vehicle in good repair for work-related travel.
Preferred Qualifications
  • CNA Certification in Palliative Care.
  • McMasters Case Management Education.
  • Meditech and Connect Care Trained.
  • Previous experience in leadership roles, program planning, education of adults, interdisciplinary practice, and research activities.
