**Job Description:**
- Provide primary health care services to individuals, communities, and specific client groups in Foam Lake and Theodore.
- Work collaboratively with other healthcare professionals to provide comprehensive care.
- Develop and implement health promotion and disease prevention programs.
- Conduct assessments, diagnose, and treat patients in accordance with evidence-based knowledge and advanced nursing practice.
- Provide education and support to patients and their families.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
**Requirements:**
- Master of Nursing - Nurse Practitioner (MN-NP) degree.
- Two (2) years of experience as an advanced RN (NP) in a relevant area.
- Three (3) years of experience as an RN in primary care, public health, or acute care.
- Five (5) years of nursing experience.
**Competencies:**
- Advanced assessment skills.
- Advanced organizational skills.
- Advanced facilitation skills.
**Knowledge and Abilities:**
- Ability to work as a member of a multidisciplinary team.
- Knowledge of health determinants and primary care principles.
- Ability to adapt to changing conditions.
