We are seeking a highly skilled and compassionate Nurse Practitioner to join our Palliative Care team at Alberta Health Services. As a Palliative Care Nurse Practitioner, you will play a vital role in providing comprehensive care to patients and their families, combining your clinical expertise with your passion for teaching, mentoring, and leading.
Key Responsibilities- Provide a full range of comprehensive health services to individuals across their lifespan, integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients.
- Utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems.
- Collaborate with interdisciplinary teams to develop and implement patient-centered care plans, ensuring seamless transitions and continuity of care.
- Participate in education, research, and leadership activities to enhance palliative care standards and contribute to the growth and development of the team.
- Work collaboratively with colleagues to provide virtual and face-to-face patient interactions, including on-call rotations to provide 24/7 virtual NP consultation coverage throughout the North Zone.
- Successful completion of an approved Nurse Practitioner program and a minimum of master's degree.
- Registration with CRNA on the Nurse Practitioner Registrar, including Provisional Graduate Nurse Practitioner licensure.
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
- Minimum of 5 years of clinical experience as a NP, with a minimum of 1 year of clinical experience managing palliative care clients (within the last 3 years) in an advance practice role.
- Minimum 1 year of Seniors Health experience and experience in at least two (2) care sectors, including Home Care.
- Advanced clinical & assessment skills, with competence in MS Office Suite, TEAMs, ZOOM, and tele-health knowledge.
- Possession of a valid driver's license and access to a motor vehicle in good repair for work-related travel, and the ability to travel in inclement weather.
- Able to carry a 30lb medical bag and/or various equipment.
- CNA Certification in Palliative Care.
- McMasters Case Management Education.
- Meditech and Connect Care Trained.
- Previous experience in leadership roles, program planning, education of adults, interdisciplinary practice, and research activities.
- A competitive rate of pay.
- A Remote Retention Allowance in place to an annual maximum of $3,000.
