Job Title: Client Care Navigator

The Client Care Navigator is a key role within Vision Loss Rehabilitation Canada, responsible for coordinating and integrating supports and services around individuals living with vision loss. This direct client service position requires strong communication and interpersonal skills, as well as the ability to work collaboratively with clients, their families, and caregivers.

Key Responsibilities:
  • Schedule appointments with primary and specialized service providers
  • Monitor and provide proactive follow-up for clients to ensure ongoing collaboration across services/providers
  • Identify opportunities for new or emerging care options to meet changing needs and address service/support gaps
  • Document all service-related activities in the client health record, following VLRC policies
  • Interact with clients, their family, and caregivers to develop and identify vision-related goals
  • Build and maintain good working relationships with professionals in other agencies and community groups
  • Educate community partners about Vision Loss Rehabilitation Canada and its specialized services offered to people with vision loss
  • Participate in multi-disciplinary team meetings
  • Proactively facilitate linkages, communication, information exchange opportunities, and coordination between clients and service providers in the healthcare system
Requirements:
  • 3-5 years of client service experience in the health and/or social service sectors
  • Experience working directly with people living with vision loss
  • Experience and knowledge in managing chronic and complex health conditions
  • Knowledge of available community services/supports and clinical, social, and residential care options
  • Understanding of roles and linkages across primary care, community care, and specialized geriatric services
  • Strong knowledge of client-centered philosophy
  • Excellent communication (verbal and written) and interpersonal skills
  • Ability to prioritize workload and manage competing tasks
  • Initiative and resourcefulness
  • Excellent problem-solving skills
  • Proficiency in technology (e.g., Microsoft Office and case management and care coordination systems)
  • Demonstrated ability to work independently and within a team
  • Bilingual (French and English)
  • Expertise and experience in cultural sensitivity and diversity
Education and Experience:
  • Registered health professional designation an asset
  • Minimum bachelor's degree in Social Work, Gerontology, or other related healthcare discipline
  • 3 or more years' experience working with individuals with vision loss
