Job SummaryWe are seeking a dedicated Youth Development Specialist to join our team at the Downtown Eastside Youth Outreach Operating team at Robert & Lily Lee Family Community Health Centre. As a Youth Development Specialist, you will play a vital role in identifying and reducing barriers that may impact youth/families' ability to access mental health supports. You will collaborate with Mental Health Clinicians on case management and outreach interventions to promote engagement and support transitions when youth age out of youth services and foster care.
About the RoleAs a Youth Development Specialist, you will:
- Provide treatment services to youth with serious mental health and substance use issues.
- Collaborate with an interdisciplinary team to manage client behavior and contribute to the development of behavioral care plans.
- Work closely with clinicians to develop, implement, maintain, and adjust treatment plans and assist clients in achieving goals outlined within their individualized treatment plan.
- Apply trauma-informed practice, harm reduction, and recovery-oriented care approaches to provide care within the interdisciplinary model.
- Co-facilitate psycho-educational groups and provide education based on theoretical models.
- Participate in team care planning for youth mental health, substance use, and concurrent disorders prevention/early intervention strategies.
- Attend case conferences/meetings with interdisciplinary team members to identify and manage youth mental health and substance use issues.
To be successful as a Youth Development Specialist, you must have:
- Baccalaureate Degree in Child and Youth Care or a Baccalaureate Degree in an equivalent appropriate health-related discipline.
- Two years recent related experience working with youth experiencing significant mental health, substance use, and/or concurrent disorder challenges in a multidisciplinary healthcare setting.
- Demonstrated knowledge of principles of recovery in mental health and substance use.
- Demonstrated knowledge of behavioral, social-emotional, and mental health/psychiatric problems in youth.
- Demonstrated knowledge and experience in Trauma-Informed Practice, Harm Reduction, and Recovery-Oriented Care.
- Demonstrated knowledge of mental health/substance use diagnoses and treatments.
- Demonstrated ability to recognize signs and indicators of clients at risk of harm to self or others and knowledge of appropriate standards/processes regarding client rights and staff obligations.
- Demonstrated knowledge of other facilities and community resources.
- Demonstrated knowledge of other healthcare disciplines and their roles in client care.
- Demonstrated knowledge of normal growth and development in children/youth.
- Demonstrated ability to work with youth experiencing behavioral, social-emotional, and mental health/substance use issues.
- Demonstrated ability to apply knowledge of theory and practice to care coordination processes.
- Demonstrated ability to communicate effectively, both orally and in writing, with youth and their families, educational personnel, resource workers, service providers, colleagues, physicians, and other healthcare staff.
- Demonstrated listening and information-seeking skills promoting communication and cooperation in problem-solving within multidisciplinary settings.
- Demonstrated ability to establish workload priorities.
- Demonstrated ability to work independently and collaboratively as a member of a multidisciplinary team.
- Demonstrated ability to provide culturally safe care to diverse populations.
- Demonstrated ability to adapt to new or unexpected events.
- Demonstrated ability to work in high-volume/high-stress environments with strong conflict resolution and problem-solving skills.
- Ability to operate related equipment.
- Physical ability to perform job duties.
- Basic computer literacy to operate computerized client care information systems and word processing, spreadsheet, internet, and email software.
