Integrated Care Lead

Job Summary:
The Integrated Care Lead is a critical member of the Integrated Care Program, responsible for providing leadership and coordination of patient care across various settings. This role requires strong communication and interpersonal skills, as well as the ability to work collaboratively with an interdisciplinary team.
Key Responsibilities:
- Act as the primary contact for patients, families, and healthcare providers to ensure seamless care transitions.
- Develop and implement patient care plans that address the unique needs of each individual.
- Collaborate with healthcare teams to ensure that patients receive comprehensive and coordinated care.
- Provide education and support to patients and families to promote self-management and independence.
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
Requirements:
- Undergraduate or graduate degree in a related health profession.
- Current membership in good standing with a relevant regulatory health professional body in Ontario.
- Five years of clinical experience, with a focus on patient-centered care and coordination.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and as part of a team.
