Job Summary

The University Health Network is seeking a highly skilled and compassionate Integrated Care Lead to join our team. As a critical member of the Integrated Care Program, you will be responsible for providing leadership and coordination of patient care across various settings, ensuring seamless transitions and exceptional outcomes.

Key Responsibilities
  • Act as the primary contact for patients, care partners, and clinical teams, facilitating communication and coordination of care.
  • Develop and implement patient care plans, ensuring comprehensive and coordinated care across acute and community settings.
  • Provide mentorship and coaching to care team members, promoting best practices and knowledge transfer.
  • Collaborate with inter-professional teams, family members, and community providers to ensure ongoing assessment and adjustment of patient care.
  • Coordinate care transitions, ensuring effective exchange of knowledge and resources to enhance patient self-management and quality of care.
  • Monitor and evaluate patient and system-level objectives, making recommendations for improvement and innovation.
  • Lead ongoing innovation and optimization efforts, synthesizing data to inform care planning and decision-making.
Requirements
  • Undergraduate or graduate degree in a related health profession.
  • Current membership in good standing with applicable regulatory health professional body in Ontario.
  • Five years of clinical experience, with a focus on community health and acute care settings.
  • Demonstrated experience and competence in patient navigation, case management, and coordination of care.
  • Excellent interpersonal, communication, and leadership skills, with the ability to work independently and collaboratively.
  • Cultural sensitivity awareness and skills to inform and best plan and facilitate coordination of patient care.
