Integrated Care Lead

Job Summary:
The Integrated Care Lead is a critical member of the Integrated Care Program at the University Health Network (UHN). This role is responsible for providing leadership and coordination of patient care across various settings, including acute care, home, and community care. The successful candidate will work closely with patients, families, and healthcare providers to ensure seamless transitions and comprehensive care.
Key Responsibilities:
- Act as the primary contact for patients, families, and healthcare providers to coordinate care and ensure seamless transitions.
- Develop and implement care plans that meet the unique needs of each patient.
- Collaborate with multidisciplinary teams to provide comprehensive care and support.
- Provide education and support to patients and families to promote self-management and independence.
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
Requirements:
- Undergraduate degree in a related health profession.
- Current membership in good standing with applicable/relevant regulatory health professional body in Ontario.
- Five (5) years of clinical experience, with a focus on patient navigation and care coordination.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a multidisciplinary team.
