About the Opportunity

The Squamish Nation is seeking a highly skilled and compassionate Nurse Assessor Leader to join our Home and Community Care (HCC) team. As a key member of our department, you will work closely with the Manager to ensure the smooth daily operations of the HCC program. Your expertise will be instrumental in providing quality home care services, implementing Client Care Plans, and promoting exceptional client service standards.

Key Responsibilities

  • Lead a team of Nurse Assessors, Senior Health Care Assistants, and Client Care Coordinators to deliver high-quality home care services.
  • Develop and implement Client Care Plans that meet the unique needs of our Nation Members and clients.
  • Collaborate with healthcare professionals to ensure seamless care coordination and access to necessary services.
  • Provide leadership and guidance to team members to promote a culturally safe and supportive work environment.
  • Stay up-to-date with British Columbia's regulations and guidelines related to Home and Community Care, ensuring compliance and best practices.
