The Squamish Nation is seeking a highly skilled and compassionate Lead Nurse Assessor to join our Home and Community Care (HCC) team. As a key member of our leadership team, you will play a critical role in ensuring the delivery of high-quality home care services to our Nation Members and clients. This is an exciting opportunity to work in a dynamic and culturally safe environment, promoting the health and well-being of our community.
Key Responsibilities
- Provide clinical leadership and guidance to HCC Nurse Assessors, Senior HCA, and Client Care Coordinator.
- Develop and implement Client Care Plans that meet the unique needs of our Nation Members and clients.
- Ensure the provision of quality home care services, adhering to British Columbia's regulations and standards.
- Collaborate with healthcare professionals to ensure seamless care transitions and coordination.
- Promote a culturally safe and supportive environment, upholding the values and customs of our Nation.

Requirements
