Physiotherapist - Clinical Expert and Community Care Coordinator

Salary
We offer a competitive salary range of $ 90,000 to 120,000 per year, commensurate with experience.
Job Description
As a Physiotherapist - Clinical Expert and Community Care Coordinator, you will be responsible for providing physiotherapy consultation, assessment, and planned interventions for diverse client groups across Southern Ontario and the Capital region. You will work within a continuous quality improvement framework that reflects VHA's vision and mission of services. Your primary goal will be to promote and maintain good public relations with clients, customers, and VHA staff, volunteers, and associates.
Key Areas of Accountability
The successful candidate will have the following key areas of accountability:
- • Assess clients using interviews, standardized and informal assessment techniques in accordance with professional practice guidelines and standards established by VHA.
• Implement client interventions based on assessment information, client input, and in conjunction with the client's integrated care plan.
• Ensure that services provided are best practice, evidence-based, and evaluated through the routine use of valid and reliable outcome measures.
• Attend client-related meetings to report on assessment findings and discuss plans for intervention, progress reports, and discharge as required.
• Ensure coordinated delivery of care through ongoing communication with all members of the client's team regarding plans and schedules for interventions.
• Document all assessment findings, treatment plans, progress, and discharge summaries of interventions as required by the profession, referral source, and the organization.
• Prepare statistical records of client care and related activities as required by VHA, the Ministry of Health, and other agencies.
Required Skills and Qualifications
To succeed in this role, you must possess the following skills and qualifications:
- • Baccalaureate or Masters degree in Physiotherapy from a recognized school of physiotherapy.
• Registered, in good standing, with the College of Physiotherapists of Ontario.
• Minimum of 2 years clinical experience, preferably in the community.
• Specialized courses to support areas of clinical expertise.
• Clinical expertise to provide a variety of physiotherapy interventions.
• Knowledge of community care requirements and resources available.
• Excellent consultation and teaching skills, and utilizes an evidence base to clinical practice.
• Familiar with the routine use of valid and reliable outcome measures to support clinical interventions.
