About the Role

We are seeking a highly skilled Palliative Care Clinical Coordinator to join our team at North York General Hospital.

Key Responsibilities
  1. Provide palliative care mentorship, education, and coaching to healthcare teams within North York Region.
  2. Establish relationships with community partners, organizations, and physicians.
  3. Establish the role of the outreach teams as an effective and efficient interprofessional Model of Care within the healthcare system.
  4. Drive practice change, with a focus on improving access to and quality of clinical care, aligned to the Palliative Care Health Services Delivery Framework in the Community.
  5. Provide palliative care coaching and mentoring to staff and clinicians in participating community organizations, to build primary-level palliative care competencies.
  6. Collaborate with leaders of community organizations on local service integration, and change management initiatives, using quality improvement methodologies to achieve results.
  7. Consult on clinical care when the complexity of the patients' needs is beyond the existing competencies of providers (in the community organizations).
Requirements
  • Regulated healthcare professional.
  • Current registration in good standing with applicable Health Care Regulated Professional College.
  • Minimum 5 years experience in palliative care.
  • Minimum 2 years experience in adult learning techniques and knowledge to practice principles.
  • Experience working with quality improvement initiatives.
  • Strong clinical coaching and debriefing skills.
  • Strong consultation skills, with expert knowledge of models of consultation and their application with individual, team and community service providers.
  • Master's degree in health care related field or equivalent, preferred.
