We are seeking a dedicated Registered Nurse to join our team in Primary Care. As a Primary Care Nurse, you will provide compassionate patient care and work collaboratively with an interprofessional team to deliver high-quality services.
Key Responsibilities
Provide clinical follow-up according to the person's primary Care Plan and personal goals of care
Develop and provide formal education to patients, families, and the general public
Work collaboratively with the interprofessional team to provide comprehensive care
Requirements
Registration with BC College of Nurses and Midwives as a practicing RN or RPN registrant
Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings
Current valid Driver's License
