Primary Care Nurse Opportunity

At Northern Health Authority, we are seeking a dedicated Registered Nurse to join our primary care team. As a Primary Care Nurse, you will play a vital role in delivering high-quality patient care and working collaboratively with our interdisciplinary team to provide comprehensive care to our patients.

Key Responsibilities:
• Provide clinical follow-up according to the patient's primary care plan and their personal goals of care.
• Develop and implement care plans in collaboration with the interprofessional team.

Requirements:
• Registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant.
• Three years of recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
• Current valid driver's license.
