Job Summary

NEXUS Community Support Society is seeking a highly motivated and compassionate Community Support Specialist to join our team. As a Community Support Specialist, you will play a vital role in supporting individuals with diverse abilities to achieve their goals and live fulfilling lives.

Key Responsibilities
  • Support and Empowerment: Develop and implement personalized support plans to empower individuals to make informed decisions and take control of their lives.
  • Community Inclusion: Facilitate community inclusion and participation in various settings, promoting social connections and a sense of belonging.
  • Documentation and Record-Keeping: Maintain accurate and up-to-date documentation of individual progress, needs, and goals.
  • Collaboration and Communication: Work collaboratively with individuals, families, and other stakeholders to ensure seamless support and communication.
  • Professional Development: Participate in ongoing training and professional development to enhance skills and knowledge in supporting individuals with diverse abilities.
Requirements
  • Age and Education: Must be 19 years of age or older and possess a high school diploma or equivalent.
  • Driving and Transportation: Hold a valid class 5 (or better) driver's license and have a reliable vehicle for transportation.
  • Experience and Skills: Related experience working with adults with diverse abilities is considered an asset. Basic computer skills and physical ability to carry out duties are required.
  • Certifications and Clearances: First Aid (Level 1) certification and a criminal record check (performed by NEXUS upon hire).
