About the Role

Fraser Health Authority is seeking a highly skilled and dedicated Mental Health Community Nurse Lead to join our team. This exciting opportunity will allow you to make a meaningful difference in the lives of individuals with mental health and/or concurrent disorders.

Key Responsibilities
  1. Care Coordination: Collaborate with interprofessional care teams to provide comprehensive treatment services, including intake functions, mental health assessments, and development of individualized care plans.
  2. Nursing Care Planning: Plan and provide direct nursing care to clients, ensuring adherence to standards of practice and best practices in mental health and substance use treatment.
  3. Treatment Services: Provide treatment services, including crisis intervention, individual counseling, and education, utilizing evidence-based approaches and theoretical models.
  4. Case Management: Initiate and participate in case conferences/meetings to coordinate client care, address emerging issues, and ensure continuity of services.
  5. Client Advocacy: Act as a client advocate, promoting autonomy, respect, privacy, dignity, and access to information.
Requirements
  • Education: Graduation from an approved School of Nursing or Psychiatric Nursing.
  • Experience: Two (2) years recent related experience providing treatment services to individuals with mental health and/or concurrent disorders.
  • Registration: Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Skills and Abilities: Strong assessment and treatment planning skills, ability to work effectively in an interdisciplinary team environment, and excellent communication and interpersonal skills.
Salary Information

The salary for this role is CAD $41.42 - $59.52 per hour, based on your qualifications and experience.
