Mental Health Treatment Coordinator

We are seeking a highly skilled Mental Health Treatment Coordinator to join our team at Fraser Health. This is a challenging and rewarding role that requires a strong background in nursing and mental health treatment.
About the Role
This position involves working with clients who have complex mental health needs, including concurrent disorders such as substance use and developmental disorders. As a Mental Health Treatment Coordinator, you will be responsible for providing treatment and care management services that are tailored to each client's unique needs.
You will work collaboratively with an interprofessional care team to provide person-centered and recovery-oriented care. Your duties will include assessing client needs, developing and implementing treatment plans, and monitoring client progress. You will also work closely with clients and their families to promote self-care and independence.
Responsibilities
- Coordinate care for clients by collaborating with interprofessional care teams, performing intake functions, conducting mental health assessments, and developing comprehensive treatment plans.
- Provide direct nursing care to clients, including administering prescribed medications and monitoring vital signs.
- Work from a strengths-based model, using motivational interviewing and engagement techniques to develop individualized care plans.
- Establish therapeutic relationships with clients and their informal support networks, promoting respect and trust.
- Provide education and support to clients and their families on managing psychiatric conditions, substance use issues, and symptoms.
- Initiate and participate in case conferences with clients, family members, service providers, and interprofessional team members to ensure coordinated care.
Qualifications
- Degree in Nursing or Psychiatric Nursing.
- Two years of recent experience providing treatment services to individuals with mental health and/or concurrent disorders.
- Valid Class 5 BC Driver's License and access to personal vehicle with appropriate insurance coverage.
Estimated Salary: $65,000 - $85,000 per year, depending on experience and qualifications. We offer a comprehensive benefits package and opportunities for professional growth and development.
