About the Opportunity

The Nova Scotia Health Authority is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in assessing, planning, and coordinating care for clients referred to Continuing Care. Your expertise will help individuals and families navigate complex health situations and access the resources they need to thrive in their communities.

Key responsibilities include:

  • Conducting comprehensive assessments to identify client needs and goals
  • Developing and implementing care plans that address client needs and promote independence
  • Authorizing services and making referrals to community resources and healthcare teams
  • Collaborating with interprofessional teams to facilitate hospital discharge and community reintegration
  • Maintaining ongoing communication with clients, families, and healthcare providers to ensure seamless care coordination

About You

To be successful in this role, you will need:

  • A Bachelor's degree in Nursing, Social Work, or a related field
  • Current registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience in a community or long-term care setting
  • Demonstrated knowledge and experience in case management or discharge planning
  • Excellent assessment, interviewing, and negotiation skills
  • Strong communication and interpersonal skills
  • Ability to work independently and collaboratively in a fast-paced environment
Hours and Compensation

This is a permanent full-time position with a 70-hour bi-weekly schedule. Shifts may include days, evenings, weekends, and holidays. The hourly rate is $41.06 - $48.39, and successful candidates may be eligible for our comprehensive benefits package.
