Job Opportunity

We are seeking a qualified Care Coordinator to join our team at Nova Scotia Health Authority.

About the Role

The Care Coordinator will work with clients and families to provide support and resources in the community, hospital, or long-term care facilities, maximizing available services to support clients and families in the home/community.

Key Responsibilities:
  • Assess clients' needs and develop care plans
  • Authorize services and make referrals to other resources
  • Work with interprofessional teams to facilitate hospital discharge
  • Provide ongoing case management and support to clients and families
  • Collaborate with clients, family, caregivers, care providers, and healthcare teams to identify and address unmet needs
About You

We are looking for a candidate with a Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies. The ideal candidate will have:

  • Current and ongoing registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience with the professional regulatory body
  • Demonstrated knowledge and/or experience in a community or long-term care setting
  • Knowledge and/or experience in dedicated case management or discharge planning
  • Excellent assessment, interviewing, and negotiation skills
  • Excellent verbal and written communication skills
  • Ability to work independently and co-operatively in a busy multidisciplinary situation
  • Ability to multi-task, prioritize workload, and manage time efficiently
  • Valid driver's license and access to a reliable vehicle
  • Competencies in other languages preferred; French an asset
Working Conditions

The successful candidate will work a permanent full-time position with 70 hours bi-weekly. Shifts may include days, evenings, weekends, and holidays.

Compensation and Benefits

The successful candidate will receive a competitive hourly rate of $48.39 and may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
