Job Title: Licensed Practical Nurse

VON Canada (Victorian Order of Nurses) is seeking a Licensed Practical Nurse to join our team in providing high-quality care to clients in their homes and communities.

Job Summary:

We are looking for a skilled and compassionate LPN to provide nursing care to clients in their homes and communities. The successful candidate will work collaboratively with our interdisciplinary team to assess, plan, implement, and evaluate client care.

Responsibilities:
  • Provide nursing care to clients in their homes and communities, including wound care, medication management, and vital sign monitoring.
  • Assess client needs and develop individualized care plans in collaboration with the interdisciplinary team.
  • Implement and evaluate client care plans, making adjustments as needed.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in ongoing education and professional development to maintain current knowledge and skills.
Requirements:
  • Current licensure as a Licensed Practical Nurse in the province of employment.
  • Experience in home care or community health an asset.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and reliable vehicle.
