About the Opportunity

The Nova Scotia Health Authority is seeking a skilled Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for assessing, planning, and coordinating the care of clients referred to Continuing Care.

Key Responsibilities:

  • Assess clients' needs and develop individualized care plans
  • Authorize services and make referrals to other healthcare professionals and resources
  • Work with interprofessional teams to facilitate hospital discharge and return to the community
  • Provide ongoing case management and support to clients and their families
  • Collaborate with community agencies and resources to ensure comprehensive care

Requirements:

  • Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience in a community or long-term care setting
  • Demonstrated knowledge and experience in dedicated case management or discharge planning
  • Excellent assessment, interviewing, and negotiation skills
  • Competent with computer use, keyboarding, and ability to conduct assessments using a laptop
  • Knowledge of community agencies and resources
