  • Key Responsibilities:

  • Develop and implement individualized treatment plans that promote recovery and wellness
  • Provide direct care to clients, including counseling, education, and support
  • Collaborate with healthcare professionals and community agencies to ensure seamless care coordination
  • Document client progress and outcomes according to practice standards and Fraser Health procedures
