About the Role

This exciting role will see you working collaboratively with an interdisciplinary team to deliver comprehensive treatment services to individuals with complex needs. You'll be responsible for assessing client needs, developing and implementing individualized care plans, and providing counseling and education to promote recovery and well-being.



Our ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams. You'll also have strong assessment and problem-solving skills, with a commitment to delivering evidence-based practice.



Responsibilities


  • Conduct initial and ongoing assessments of clients' mental health and substance use needs
  • Develop and implement comprehensive treatment plans that address physical, emotional, and social needs
  • Provide counseling and education to clients and their families on topics such as coping strategies, relapse prevention, and medication management
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and transitions
  • Maintain accurate and timely documentation of client progress and outcomes


Requirements


  • Diploma or degree in nursing (RPN/RN)
  • Two years of recent experience in mental health and/or substance use nursing
  • Current practicing registration with the BC College of Nurses and Midwives
  • Valid Class 5 BC driver's license and access to a reliable vehicle
  • Ability to communicate effectively in English (written and verbal)
