Job Title: Social Worker

Rehab First Inc. is seeking a skilled Social Worker to join our team of healthcare professionals.

About the Role

We are looking for a highly motivated and experienced Social Worker to provide psycho-social assessments, case management, and counseling services to our clients. The successful candidate will have a Master's degree in Social Work and current registration with the Ontario College of Social Workers and Social Service Workers.

Key Responsibilities
  • Conduct comprehensive psycho-social assessments to identify client needs and develop OCF-18 plans under Ontario's Statutory Accident Benefit regulations
  • Provide counseling services in various settings, including in-person, telephone, and cyber counseling
  • Develop and implement case management plans to support clients in achieving their goals
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document client progress and maintain accurate records
Requirements
  • Master's degree in Social Work (MSW)
  • Current registration with the Ontario College of Social Workers and Social Service Workers
  • Experience in a healthcare setting, preferably in the community
  • Familiarity with relevant legislation, including the Health Care Consent Act and Mental Health Act
  • Excellent communication, problem-solving, and time management skills
  • Self-motivated and able to manage a busy caseload
