About the Role

We are seeking a highly skilled and compassionate Social Services Professional to join our team at the Hamilton Family Health Team. As a key member of our interprofessional team, you will play a vital role in providing clinical support to patients and families, promoting health and wellness, and advocating for the needs of our community.

Key Responsibilities
  • Biopsychosocial Assessments: Conduct thorough assessments to identify patients' needs and develop personalized treatment plans.
  • Treatment Interventions: Provide short-term treatment, evaluation, and resource referrals as needed.
  • Collaboration and Communication: Work closely with patients, primary care providers, and interprofessional teams to monitor and adapt treatment plans.
  • Health Promotion and Education: Develop and implement health promotion activities to increase outreach and patient enrollment.
  • Case Management: Assist patients and providers in navigating the healthcare system, identifying and connecting them with appropriate services and supports.
  • Team Building and Quality Improvement: Participate in team building, program development, and quality improvement activities to enhance patient care and outcomes.
  • Advocacy and Liaison: Advocate for patients and their families, and liaise with other healthcare providers and community agencies as needed.
Requirements
  • Master of Social Work: Possess a Master's degree in Social Work from an accredited institution.
  • Membership in Good Standing: Be a member in good standing with the Ontario College of Social Workers and Social Service Workers.
  • Professional Liability Insurance: Possess professional liability insurance.
  • Experience in Healthcare Settings: Have experience delivering social work and mental health services in healthcare settings.
  • Interprofessional Team Experience: Have experience working on interprofessional teams in healthcare settings.
  • Health Promotion Experience: Possess experience in health promotion and education.
  • Strong Understanding of Legislation: Have a strong understanding of legislation related to healthcare, such as PHIPA.
  • Strong Understanding of Social Work Values and Ethics: Possess a strong understanding of social work values and ethics in clinical practice.
