We are seeking a highly skilled and compassionate Social Worker to join our team at Rehab First Inc.
Key Responsibilities- Conduct thorough psychosocial assessments to identify client needs and develop effective care plans.
- Assign and manage case loads, ensuring timely and high-quality service delivery.
- Develop and implement OCF18 plans in accordance with Ontario's Statutory Accident Benefit regulations.
- Provide counseling services to clients in various settings, including in-person, phone, and online sessions.
- Maintain accurate and detailed documentation of client interactions and progress.
- Collaborate with multidisciplinary teams to ensure comprehensive and coordinated care.
- Opportunity to work with diverse populations, including adults and children.
- Master's degree in Social Work (MSW) from an accredited institution.
- Current registration with the Ontario College of Social Workers and Social Service Workers.
- Experience working in healthcare settings, preferably in community-based roles.
- Familiarity with relevant legislation, including the Health Care Consent Act and Mental Health Act.
- Excellent communication, problem-solving, and time management skills.
- Self-motivated and able to manage a busy caseload.
- Valid driver's license, auto insurance, and access to a reliable vehicle.
- Completion of a satisfactory Police Vulnerable Sector Check within the last six months.
