Job Summary

Hamilton Health Sciences is seeking a skilled Social Worker to join our team. As a member of our healthcare team, you will provide direct patient care through psycho-social screening and assessments, individual and family counseling, crisis intervention, bereavement counseling, liaison and referral to community agencies, advocacy, and discharge planning.

Key Responsibilities
  • Provide psycho-social screening and assessments to patients and families
  • Offer individual and family counseling to address mental health and wellness needs
  • Develop and implement crisis intervention plans to support patients and families
  • Provide bereavement counseling to patients and families
  • Liaise and refer patients and families to community agencies and resources
  • Advocate for patients and families to ensure their needs are met
  • Develop and implement discharge plans to support patients and families
Qualifications
  • Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker
  • Baccalaureate degree in Social Work from an accredited School of Social Work, Masters degree in Social Work preferred
  • Previous crisis intervention experience
  • Expert knowledge of community resources
  • Demonstrated skills in the performance of a therapeutic role, including strong assessment, counseling, case management, and advocacy skills
  • Demonstrated strong interpersonal, teamwork, and communication skills
Working Conditions

As a Social Worker at Hamilton Health Sciences, you will work in a fast-paced environment with a diverse patient population. You will be required to work days, evenings, nights, weekends, and holidays. You will also be required to work in a team environment and communicate effectively with patients, families, and healthcare staff.
