Job Title: Social Worker MSW

Job Summary:

The McMaster Family Health Team (MFHT) is seeking a skilled Social Worker to join our interdisciplinary primary care team. As a Social Worker, you will provide direct patient care through psycho-social assessments, individual, couple, family, and group counseling. You will work closely with physicians, family medicine residents, psychiatrists, nurse practitioners, and other allied health professionals to provide comprehensive care to patients.

Key Responsibilities:

  • Provide psycho-social assessments and develop treatment plans for patients
  • Conduct individual, couple, family, and group counseling sessions
  • Consult with physicians and other healthcare professionals to coordinate patient care
  • Triage patient referrals and manage a waiting list
  • Develop and maintain relationships with community social workers and programs to support patients
  • Tutor family medicine residents, social work students, and other health professional students
  • Participate in interdisciplinary team development, continuing education activities, and primary health care research

Requirements:

  • Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker
  • Masters degree in Social Work from an accredited School of Social Work
  • Recent (3-5 years) experience providing individual, marital, family, and group counseling
  • Demonstrated knowledge of suicidal risk assessment, crisis management, and verbal diffusion techniques
  • Working knowledge of the Mental Health Act, Healthcare Consent Act, Substitute Decisions Act, and community mental health resources
  • Demonstrated skills in the performance of a therapeutic role, including strong assessment, counseling, case management, and advocacy skills
  • Strong interpersonal, team work, and communication skills
  • Demonstrated interest in teaching social work students at the undergraduate and/or graduate level
  • Evidence of ability to develop and conduct educational programs for learners, and experience in evaluation of learning activities specific to individual performance
