As a Mental Health Services Coordinator, you will play a crucial role in coordinating Specialized mental health and substance use services, ensuring seamless transitions between care settings and promoting patient-centered care.

In this role, you will:

  • Coordinate and assign requests for consultation
  • Prioritize consultations based on urgency and patient assessment
  • Provide liaison support with physicians, nursing unit staff, and other healthcare professionals
  • Support care planning and education for healthcare professionals
  • Network with community programs and facilitate coordinated transitions to community services

We offer a competitive salary range of $85,000 - $110,000 per annum, commensurate with experience, as well as comprehensive benefits and opportunities for growth and development.
