Job Title: Forensic CommunityAbout the Role

This position provides comprehensive nursing care to individuals with severe and persistent psychiatric illnesses within the community. The successful candidate will work collaboratively with mental health teams, private psychiatrists, and social service agencies to ensure continuity of care and sharing of pertinent information.

Key Responsibilities
  • Conduct initial intake and ongoing biopsychosocial nursing psychiatric assessments, including current mental status examinations and general assessments of physical health.
  • Develop and implement integrated treatment plans based on clinical risk and need assessments.
  • Provide community case coordination and advocacy services for clients, including communication and monitoring of client contacts with mental health teams, private psychiatrists, and social service agencies.
  • Maintain concise and accurate documentation of relevant information on client files in accordance with policy standards.
  • Provide nursing services, information, and support, as well as short-term, time-limited follow-up as required.
Requirements

The successful candidate will possess the following qualifications and skills:

  • Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse or Registered Psychiatric Nurse.
  • Two years recent, related experience working with individuals in the community with severe and persistent psychiatric illness.
  • Broad knowledge of nursing theory and Forensic Psychiatric Nursing practice.
  • Broad knowledge of other facilities and community resources.
