About the Opportunity

At Nova Scotia Health, we're on a mission to achieve excellence in health, healing, and learning through collaboration and teamwork. As a key member of our interdisciplinary healthcare team, you'll provide collaborative, holistic care on the primary to quaternary care continuum.

The ideal candidate will be a skilled Health Care Social Worker with experience in adult community mental health, who can build upon patients' strengths, focusing on existing coping strategies, problem-solving skills, past successes, and personal resources.

Key Responsibilities
  • Engage with patients/clients to assess their needs and develop comprehensive care plans
  • Build relationships with community partners to enhance care delivery
  • Conduct biopsychosocial assessments to inform treatment decisions
  • Facilitate group sessions using evidence-based approaches like Psychosocial Rehabilitation and Motivational Interviewing

We're looking for a professional with a strong understanding of social determinants of health, excellent communication skills, and the ability to work collaboratively within our dynamic team environment.
