About the Opportunity

The Continuing Care Coordinator plays a vital role in assessing, planning, and authorizing services to support community-dwelling clients. This position involves collaborating with interprofessional teams to facilitate hospital discharge and ensure a smooth transition back to the community.

Care Coordinators work closely with clients, family members, caregivers, and healthcare providers to identify goals, unmet needs, and resource requirements. Based on the assessment, they assist in formulating a plan to meet identified objectives and provide ongoing case management to ensure effective care delivery.

This role requires strong assessment, interviewing, and negotiation skills, as well as excellent verbal and written communication skills. The ideal candidate will be able to work independently and collaboratively in a busy multidisciplinary setting, prioritizing workload and managing time efficiently.

Key Responsibilities
  • Assess client needs and develop personalized care plans
  • Authorize services and make referrals/linkages to other professional and volunteer resources
  • Collaborate with interprofessional teams to facilitate hospital discharge
  • Provide ongoing case management to ensure effective care delivery
Required Qualifications
  • Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy or Nutritional Studies
  • Current and ongoing registration (or eligibility for registration) with the applicable Nova Scotia Professional Regulatory body
  • Minimum three (3) years' experience with the professional regulatory body
  • Demonstrated knowledge and/or experience in a community or long term care setting required
Salary and Benefits

$43.35 - $49.36 hourly
