Job Summary

We are seeking an experienced Registered Nurse to join our Home Health team as a Community Health Nurse. As a key member of our team, you will provide comprehensive nursing care to clients with acute, chronic, and palliative health needs in their homes and communities.

Key Responsibilities
  • Care for clients with complex health needs, including those requiring support with daily living activities, medication management, and wound care.
  • Work collaboratively with clients, families, and healthcare professionals to develop and implement personalized care plans that promote independence and improve quality of life.
  • Conduct thorough assessments of client needs and preferences, and coordinate services to meet those needs.
  • Provide education and support to clients and their families on self-management techniques, disease prevention, and health promotion strategies.
  • Collaborate with other healthcare professionals to ensure seamless transitions of care and optimal health outcomes.
Requirements
  • Diploma or degree in Nursing from a recognized institution.
  • Current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).
  • Minimum one year of recent experience in a clinical setting, preferably in community health nursing or a related field.
  • Strong assessment and intervention skills, with ability to prioritize and manage multiple clients and tasks simultaneously.
  • Excellent communication and interpersonal skills, with ability to work effectively with diverse populations and healthcare teams.
