As a Community Liaison Worker with our Integrated Care Team at Heatley Community Health Centre in Vancouver, you will play a vital role in supporting clients with mental health and substance use/addiction issues.
Salary
The estimated salary for this position is CAD $32.42 per hour, ranging from CAD $31.56/Hr. to CAD $33.28/Hr.
Job Description
We are seeking a skilled Community Liaison Worker to join our team. Key responsibilities include:
- Supporting an integrated health service team and delivery model.
- Providing support services to clients with mental health and substance use/addiction issues.
- Maintaining client records and providing verbal reports as needed.
- Facilitating access to community resources and services.
- Liaising with healthcare professionals to coordinate care.
- Planning, organizing, and facilitating groups and case planning.
- Acting as an advocate for clients and providing resource information.
Required Skills and Qualifications
To be successful in this role, you must have:
- A certificate from a recognized Health Care Worker program, such as the Community Health Worker Program, plus two years of related experience.
- Valid driver's license and access to a personal vehicle for business purposes.
- Knowledge of community, social, and health resources.
- Ability to communicate effectively and work collaboratively with others.
- Physical ability to perform the duties of the position.
