Care Transitions Coordinator- Patient Flow Specialist

We are seeking a highly skilled Care Transitions Coordinator- Patient Flow to join our team at North York General Hospital. As a key member of our Patient Flow department, you will play a critical role in ensuring the smooth transition of patients through our hospital, from admission to discharge.
Key Responsibilities- Coordinate and facilitate patient admissions, discharges, transfers, and transitions, utilizing our hospital's flow and access tools, standards, and protocols.
- Work collaboratively with physicians, Bed Control Specialists, Social Workers, LHINs, community providers, and other stakeholders to ensure timely and appropriate patient placement.
- Assess patient transition needs and challenges, and identify and mitigate barriers to discharge and care transitions.
- Support discharge planning and workflow optimization for Resource Matching and Referrals, iPlan Platform, Alternate Level of Care patients, and integrated care models.
- Capture and track data related to patient flow and transitions operations, and develop KPIs to evaluate performance.
- Verify, correct, and submit data and compliance reports pertaining to patient flow activities, occupancy, ALC, etc.
- Undergraduate degree in a healthcare discipline.
- Current membership and good standing with a professional college.
- Minimum 3 years' recent clinical experience in an acute care hospital setting.
- Project Management (PMP Certification) and Lean Six Sigma certification are preferred.
- Excellent communication, facilitation, and conflict resolution skills.
- Ability to work independently and effectively to assess patient placement needs.
- Proven leadership skills and data quality management experience.
