Key Responsibilities
In this role, you will:
Coordinate patient admissions, discharges, and transfers, utilizing established protocols and tools to ensure efficient patient flow. Collaborate with clinical and non-clinical teams to facilitate patient placement, leading daily meetings to enhance the patient experience. Assess patient transition needs and work to eliminate barriers to discharge and care transitions, engaging with various healthcare professionals. Support discharge planning and optimize workflows for Resource Matching and Referrals, as well as manage Alternate Level of Care patients. Track and analyze data related to patient flow operations, developing key performance indicators to assess effectiveness. Prepare and submit compliance reports regarding patient flow activities and occupancy as required. Lead and participate in quality improvement initiatives related to Patient Flow and Care Transitions.
Qualifications
To be successful in this position, you will need: An undergraduate degree in a healthcare discipline. Current registration with a professional college. A minimum of three years of recent clinical experience in an acute care setting. Preferred certifications in Project Management (PMP) and Lean Six Sigma. Strong leadership and facilitation skills, with the ability to manage conflict effectively. Proficiency in Microsoft Office applications. Bilingualism in French and English is an asset.
