Patient Flow Care Transitions Specialist

Job Overview
The Patient Flow Care Transitions Coordinator plays a crucial role in ensuring the seamless movement of patients through various stages of care. This position is dedicated to coordinating and facilitating all aspects of patient transitions, from admission to discharge, in collaboration with allied health professionals. The ultimate aim is to guarantee that patients receive timely, safe, and integrated care throughout their healthcare journey.
Key Responsibilities
- Coordinate patient admissions, discharges, transfers, and transitions using established tools and protocols, collaborating with physicians, social workers, and community providers.
- Lead daily patient placement meetings to enhance the patient experience, focusing on timely access to inpatient beds and overall patient flow.
- Assess patient transition needs and challenges, working with various care teams to identify and address barriers to effective discharge and transitions.
- Support discharge planning and optimize workflows for Resource Matching and Referrals, working closely with clinical professionals.
- Track and analyze data related to patient flow and transitions, developing key performance indicators to evaluate operational effectiveness.
- Ensure compliance by verifying and submitting reports related to patient flow activities and occupancy rates.
- Drive continuous quality improvement initiatives related to patient flow and care transitions.
Qualifications
- A bachelor's degree in a healthcare discipline is required.
- Must be a current member in good standing with a professional college.
- A minimum of three years of recent clinical experience in an acute care hospital setting is essential.
- Project Management Professional (PMP) certification is preferred.
- Lean Six Sigma certification is an asset.
- Strong leadership, facilitation, and conflict resolution skills are necessary.
- Proficiency in Microsoft Office applications is required.
- Bilingualism in French and English is considered an asset.
