Patient Flow Manager

We are seeking a highly skilled and experienced Patient Flow Manager to join our team at the University Health Network. As a key member of our operations team, you will play a critical role in ensuring the smooth and efficient flow of patients through our healthcare system.
The successful candidate will have a strong background in healthcare operations, with a focus on patient flow and discharge planning. You will be responsible for developing and implementing strategies to improve patient flow, reducing length of stay and readmissions, and enhancing the overall patient experience.
As a Patient Flow Manager, you will work closely with our clinical teams, including physicians, nurses, and other healthcare professionals, to ensure that patients receive the highest quality care in a timely and efficient manner. You will also be responsible for collaborating with our community partners and stakeholders to ensure seamless transitions of care.
We are looking for a dynamic and process-oriented individual who is passionate about improving patient care and outcomes. Key Responsibilities- Develop and implement strategies to improve patient flow and reduce length of stay
- Collaborate with clinical teams to ensure timely and efficient discharge planning
- Work with community partners and stakeholders to ensure seamless transitions of care
- Monitor and analyze patient flow data to identify areas for improvement
- Develop and implement process improvements to enhance patient experience and outcomes
- Bachelor's degree in a related field (e.g. healthcare administration, business, etc.)
- Minimum 4 years of experience in healthcare operations, with a focus on patient flow and discharge planning
- Strong analytical and problem-solving skills
- Excellent communication and collaboration skills
- Ability to work in a fast-paced environment and prioritize multiple tasks and projects
