We are seeking an experienced Registered Practical Nurse (RPN) to join our team as a Healthcare Team Lead in our eHomecare program. As a key member of our team, you will be responsible for supervising and directing the care delivery of our healthcare professionals, ensuring that clients receive safe and quality care in their own homes.

Key Responsibilities:

  • Supervise and direct the work of healthcare professionals to ensure compliance with VON policies and procedures
  • Collaborate with the service delivery team to deliver high-quality client care plans
  • Provide training and education to healthcare professionals on best practices and new technologies
  • Ensure timely completion of client assessments and care plans
  • Monitor and evaluate the effectiveness of client care plans and make recommendations for improvement
  • Maintain accurate records and reports as required

Requirements:

  • Registered Practical Nurse (RPN) diploma
  • Minimum 5 years' experience in community-based healthcare
  • Current registration with the College of Nurses in the province of practice
  • Team lead or supervisory experience preferred
  • Strong observation and assessment skills
  • Advanced computer skills, including Microsoft Office Suite and Windows OS
  • Strong verbal and written communication skills
  • Interpersonal relationship building
  • Ability to problem-solve and adapt nursing interventions as appropriate
