Nurse Team Lead Position for Community Care

The Nurse Team Lead plays a crucial role in ensuring the delivery of high-quality patient care in our community-based healthcare system.
About the RoleWe are seeking an experienced Registered Practical Nurse (RPN) to lead a team of healthcare professionals in providing comprehensive care to clients. The successful candidate will be responsible for coordinating and supervising the work of the team, as well as facilitating communication among team members and management.
Responsibilities- Supervise the work of the team according to established guidelines and geographical boundaries.
- Work with team members and management to ensure adequate staffing requirements are met.
- Hold regular team meetings and huddles to keep team members informed of key performance indicators and essential information for excellent service delivery and team effectiveness.
- Utilize effective communication skills to obtain necessary information from clients, families, and healthcare team members.
- Provide complete and relevant assessments of clients and their environments using established procedures.
- In collaboration with the healthcare team, contribute to the development and implementation of client care plans.
- Document identified problems using a specified framework.
- Select and implement strategies or interventions from established care plans in consultation with clients, caregivers, and healthcare team members.
- Description, evaluation, and documentation of client responses to care using established procedures.
- Maintain adherence to best practice guidelines at all times.
- Apply adult learning principles and established training materials to teach and enhance skill levels of service providers, including Client Care Coordinators as needed.
- Provide hands-on personal care as required.
- Conduct home visits with service providers to assess quality service and competency, and ensure optimal performance levels meet client service requirements.
- Receive and document employee, family, and client complaints/issues related to service delivery, and take appropriate steps to achieve satisfactory resolution.
- Met with individual team members to build education and learning plans, and provide input to annual Performance Development Plans (PDPs).
- Act as a clinical resource to the healthcare team.
- Demonstrate effective time management, planning, and organization of daily work activities and team member responsibilities.
- In collaboration with the Client Care Coordinator, assign visits to team members as needed, considering urgent care requests, missed visits, and additional visits.
- Advocate on behalf of clients to ensure the delivery of quality care, adhering to established guidelines.
- Promote client-directed care, self-care, and independence through teaching.
To be successful in this role, you must possess:
- A Registered Practical Nursing (RPN) diploma.
- A minimum of 5 years' experience in a community-based healthcare system.
- Registration with the College of Nurses in your province of practice.
- Team lead or supervisory experience is preferred.
- Strong nursing skills, including the ability to read and interpret technical and clinical instructions.
- Clinical practice experience is desirable.
- Current CPR certification.
- Experience in health teaching and facilitation is preferred.
