Job Summary

We are seeking a skilled Registered Practical Nurse to join our Community Visiting Nursing Program at Qualicare, Burlington. As a visiting nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our team to deliver exceptional patient outcomes.

Key Responsibilities
  • Provide comprehensive nursing care to patients in their homes, including wound care, IV management, and diabetic care.
  • Conduct thorough assessments and develop individualized care plans to meet the unique needs of each patient.
  • Collaborate with our multidisciplinary team to ensure seamless care coordination and communication.
  • Participate in ongoing education and professional development to stay up-to-date on best practices and industry advancements.
Requirements
  • Current registration with the College of Nurses of Ontario (CNO) and in good standing.
  • Experience in Home and Community Care is an asset.
  • Current BCLS certification (Level C).
  • Demonstrated clinical assessment and patient/family teaching skills.
  • Demonstrated effective communication skills.
  • Charting on Procura is an asset.
