Job Title: Registered Practical Nurse - Home Care

Bayshore HealthCare is seeking a skilled Registered Practical Nurse to join our team in the Greater Hamilton Area. As a key member of our home care team, you will provide high-quality nursing care to clients in their own homes, promoting independence and improving their overall well-being.

Key Responsibilities:
  • Provide skilled clinical care to clients in their homes, including palliative, pediatric, wound care, and IV therapy.
  • Develop and implement individualized care plans to meet the unique needs of each client.
  • Collaborate with clients, families, and healthcare teams to ensure seamless care transitions and optimal outcomes.
  • Conduct thorough assessments, identify areas for improvement, and implement evidence-based interventions.
  • Delegate tasks and functions to different levels of workers and client family members, promoting autonomy and independence.
Requirements:
  • Current registration as a Registered Practical Nurse with the College of Nurses of Ontario.
  • Valid driver's license and reliable vehicle.
  • Updated immunization records, clear criminal background check, and vulnerable sector check upon hire.
  • Current CPR certification.
  • Leap Training and Palliative Certificate considered an asset.
