About the Role

CarePartners is seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our community nursing team. As a Community Nurse, you will have the opportunity to provide high-quality patient care in a variety of settings, including in-home, clinics, and retirement homes.

Key Responsibilities

  • Develop and maintain effective care plans for patients receiving community health services
  • Provide exceptional nursing services to patients, both in-person and virtually
  • Communicate and liaise with patients, family members, and other healthcare professionals
  • Perform other clinical duties as required

Requirements

  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Valid driver's license and vehicle
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record, including 2-step TB test
