Nurse - Community Health Specialist

About the Role
CarePartners is seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our Community Nursing Services team. As a Community Nurse, you will have the opportunity to work in a variety of settings, including in-home, clinics, and retirement homes, providing exceptional nursing services to patients in need.
Key Responsibilities
- Develop and maintain effective care plans for patients receiving Community Health Services
- Provide nursing services to patients in their homes, clinics, and other settings
- Communicate and collaborate with patients, family members, and other healthcare professionals to ensure seamless care
- Perform other clinical duties as required

Requirements
- Current College of Nurses of Ontario (CNO) registration in good standing
- Valid driver's license and vehicle
- Excellent communication and interpersonal skills
- CPR Certification and current immunization record
