Nurse - RPN Community Care Specialist

We are seeking a skilled Registered Practical Nurse (RPN) to join our team as a Community Care Specialist. In this role, you will provide high-quality nursing care to patients in their homes, working collaboratively with our team to ensure the best possible outcomes.
Key Responsibilities- Conduct scheduled visits in the community to assess patient needs and provide care within your scope of practice.
- Develop and implement individualized care plans, working closely with patients and their families to ensure their needs are met.
- Provide education and support to patients and their caregivers, promoting independence and self-management.
- Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
- Maintain accurate and up-to-date patient records, documenting all interactions and care provided.
- Current registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse in good standing.
- Valid driver's license and access to a reliable vehicle.
- Experience in community care is an asset, but not required.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
