Clinical Nurse Specialist

Job Summary:
We are seeking a skilled and compassionate Clinical Nurse Specialist to join our team at ParaMed Home Health Care. As a key member of our nursing staff, you will play a vital role in delivering high-quality patient care and supporting our mission to provide exceptional healthcare services.
Key Responsibilities:
- Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
- Develop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care coordination.
- Collaborate with multidisciplinary teams to identify and address patient needs, and develop effective care plans.
- Conduct regular home visits to assess patient needs, provide education, and support, and make recommendations for care.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Stay up-to-date with industry developments, best practices, and regulatory requirements to ensure compliance and excellence in care delivery.
Requirements:
- Current registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse (RPN) in good standing.
- Valid driver's license and access to a reliable vehicle for travel to patient residences.
- Experience in delivering community care, preferably in a home health setting.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and as part of a team, with a strong focus on patient-centered care.
- Flexibility to work varied shifts, including evenings and weekends, and to travel to patient residences within a designated area.
