Job Summary:

We are seeking a skilled Registered Practical Nurse (RPN) to join our team as a Community Nursing Role - RPN Position. In this role, you will provide high-quality nursing care to patients in their homes, working collaboratively with our team to ensure the best possible outcomes.

Key Responsibilities:

  • Participate in scheduled visits to assess patient needs and provide care within your scope of practice.
  • Provide safe and ethical nursing care, using appropriate assessment tools and techniques to assess patients' individual needs.
  • Execute and continuously monitor care plans, adjusting as required to ensure optimal patient outcomes.
  • Engage patients using a collaborative approach, building rapport and trust to promote positive relationships.
  • Maintain and update professional patient records related to patient encounters and plans.

Requirements:

  • Registration with the CNO as a Registered Practical Nurse in good standing.
  • Valid driver's license and access to a vehicle.
  • Experience in delivering community care is an asset.
  • Reliability and flexibility.
  • Willingness to travel to and from patient residences within a designated area.
  • Experience working with vulnerable populations.
  • Passion for making a difference in your community.
