Mental Health Social Worker

We are seeking a highly skilled and compassionate Social Worker to join our team at Fraser Health Authority. As a key member of our mental health services team, you will play a vital role in providing clinical social work services to clients and their families.
Key Responsibilities- Provide comprehensive therapeutic clinical counselling services to clients and their families, community supports, and interdisciplinary treatment teams.
- Maintain an active role in planning for care during the hospital stay, and facilitating optimal discharge planning by interviewing the client and/or family to assess and determine a care plan and type of service required.
- Conduct comprehensive social work and psychosocial assessments of clients, and families by interviewing the client and/or family, obtaining relevant information, gathering social data regarding the client and/or family and interpreting the data using a variety of modalities.
- Educate and provide consultation and support to clients, families, and healthcare professionals on issues related to high risk factors, recovery and current community practice in the field of mental health, child protection, adaptation to illness and disability, protection of vulnerable adults, chemical dependency, community living and support services.
- Master's Degree in Social Work (MSW) from an accredited school of Social Work.
- Two (2) years' recent, related experience in a mental health treatment setting or an equivalent combination of education, training, and experience.
- Current full registration with the British Columbia College of Social Workers.
- Valid Class 5 BC driver's license with access to personal vehicle for business purposes.
